You can intubate a patient. You can read an ECG at a glance. And yet a German Stationsübergabe — the five-minute morning handover — can leave you standing there with nothing but ja and a hopeful smile.
Healthcare German is not general German with longer words glued on. It is its own register: clipped, fast, full of abbreviation, and far less forgiving of a misheard dosage than a misheard restaurant order.
Here is what each profession actually needs, the vocabulary that matters, and how long it takes to get there.
What you will get from this guide
- Level requirements for nurses, doctors and allied health roles
- Which exams count: Goethe B2, telc Pflege, the Fachsprachprüfung
- High-frequency vocabulary for wards, symptoms and handovers
- A realistic timeline from A2 to B2 for working professionals
The Levels: What Each Profession Needs
Requirements differ by profession, by Bundesland and by the recognition authority handling your case. But the pattern is consistent enough to plan around.
Two warnings. First, the certificate is not the goal — the Anerkennung (recognition) is. Second, "B1 required" rarely means B1 is comfortable in a real ward. Ask the authority in writing and confirm with an official source such as anerkennung-in-deutschland.de.
Exam Options, Compared Honestly
The pattern to notice: a general B2 certificate opens the door, then a profession-specific exam decides whether you walk through it.

The Vocabulary That Gets You Through a Shift
Start with the body and the complaint.
Then the ward and the workflow.
Then the phrases you will say fifty times a day.
General B2 is not clinical B2
Standard courses teach you to order bread, not to hand over a deteriorating patient at 6 a.m. Our German course pass adds healthcare-focused speaking practice with an AI voice tutor, plus printable study materials for telc Pflege, Goethe B2 and clinical interviews.

Clinical German runs on abbreviation and speed.
You are not expected to decode this in week one. You are expected to decode it before you are alone on a shift.
A Realistic Timeline for Working Adults
Ten hours a week is the honest minimum for a job deadline. Below that, you collect vocabulary and lose motivation.
Where Candidates Actually Fail
Passing B2, then freezing at the Visite. Certificates test general German. Visits test interrupted, dialect-flavoured, high-stakes speech.
Learning nouns only. A ward shift is verbs and instructions: anhängen, abnehmen, umlagern, durchführen.
Skipping written documentation. Pflegedokumentation is graded in several recognition processes and is where grammar mistakes become legally visible.
Ignoring regional dialect. A Bavarian ward and a Berlin ward sound like two different countries. Exposure beats theory.
Assuming the certificate is the last step. For doctors, the Fachsprachprüfung is a separate performance exam run by the relevant Landesärztekammer. Plan for it from day one.

What level of German do foreign nurses need in Germany? Commonly B1 to B2, and many employers expect B2 in practice. Exact requirements vary by Bundesland and recognition authority, so confirm your specific case before booking an exam.
Is Goethe B2 enough for doctors? It is usually the base requirement, but most Länder also require the Fachsprachprüfung, a clinical performance test. You need both language proof and clinical communication.
Should I take telc Pflege or general Goethe B2? If your profession is nursing and your authority accepts it, telc Deutsch B1–B2 Pflege is closer to the job. General B2 is more portable if you may switch fields.
How long from A2 to B2 while working full time? Roughly 20 to 40 weeks at 10 to 20 hours a week. Fewer than 10 hours a week makes the finish line recede faster than you approach it.
