A standard prescription has to be started within fourteen days of the date it was written, and one marked with a dringender Behandlungsbedarf is expected to begin almost at once. Physiotherapists in Germany rarely change a prescription on their own authority, and when the prescribed treatment no longer fits, the correct step is a Rücksprache with the doctor who issued it.
