In the Netherlands the huisarts, the family doctor, is the centre of the system. You cannot walk straight into a specialist; you go to the practice first, and the practice decides what happens next.

The assistente answers the telephone and decides how quickly you are seen, so describe your complaint in one clear sentence at the start of the call rather than saving it for the end.

Call the practice at eight in the morning for a same-day appointment, and use the huisartsenpost in the evening or at the weekend. For a life-threatening emergency, call 112 and not the practice.

Save the number of the practice in your phone, and take both your identity document and your insurance card to the first visit. The doctor records your history once and uses it for every later appointment.

You report sick to your employer, not to the doctor, and employers must keep paying most of your wage in the first year. The company doctor guards the process and may invite you for a talk about returning.

The pharmacy keeps your medication history, so use the same one and ask for the generic version. For a minor complaint the assistant may advise paracetamol and rest, which is standard Dutch practice and not a refusal to help.

A hospital visit normally begins with a verwijsbrief from the practice, except in an emergency, when you go straight to the spoedeisende hulp. Take your insurance card, because it is asked for at the desk.

A short, concrete sentence is all the doctor needs, and Dutch consultations are famously direct. Prepare two or three of these phrases and the appointment takes five minutes instead of twenty.

Choosing and Using Your Insurance

A Dutch health policy is personal, so every adult takes out and pays for their own cover, and households on a low income may claim the zorgtoeslag to reduce the premium.

You may switch insurer at the end of each calendar year, and the new policy begins on the first of January if you cancel before the deadline at the end of December.

The excess is paid from the first care you receive, except for visits to the family doctor, which are always free, and for children, who pay no excess at all.

Supplementary packages cover dentistry, glasses and physiotherapy, and they are worth comparing only after you have decided on the basic policy that suits your health.

The insurer also handles the reimbursement of hospital bills, so keep every invoice and check the annual statement for care you do not recognise.

If you work and your income changes, report it through the tax office, because the allowance is corrected every year against what you actually earned.
